On a related note, does anyone know the syntax (if it's possible) to allow access to a page only if the referring page is one specified?

i.e can get to www.aaa.com if coming from www.bbb.com or www.ccc.com only?

Why not just use an online one with a sample username/password combination (in case you're concerned about typing out a username/password onto another site) then just edit the file before uploading it to your site.

Its not that - i am just after an offline one to stick on an offline server
