Well if you count a random decision as one made without method or conscious decision then that's pretty much impossible. If you ask a human to do it then they'll just come up with a number that they think is random, based on whatever arbitrary reason they have for thinking that number is a bit odd, or what they chose last time they were asked for a random number, or any other number of factors.

If you ask a computer (which is just asking a human in a roundabout sort of way) then it will just go through an algorithm with a specific number (usually the number of seconds since Jan 1st, 1970) and give you the result. Neither of these options will give you a truly random number.

That said... 4.
