Old Three GS2 - T mobile sim

Planning on giving my old GS2 to my dad. I bought it with a 3 contract back in 2011.

He has just tried to put his T mobile sim in the phone and upon boot up I get a 'enter sim network unlock PIN' request.

Do I get this from 3 or from T mobile?

Conciderig the phone is fully payed off and I am not longer in contract would 3 provide an unlock code?

Also can it be done else where?

Three now unlock their phones free of charge even if still in contract, and all new phones are unlocked out the box

One question I've got tho is on the website it wants me to put my existing phone number and the IMEI of the old handset.

The problem is I have had my number ported over to a new sim as I now have a Nexus 5.

Does it matter?
