Online survey..any easy way to make one?

Hello all.

I am quite a noob when it comes to this sort of stuff….i am hoping I can get some help from you kind chaps.

I think I may need to make an online survey soon, all I am looking for is something quite simple with tick boxes then a submit button that sends results to email.

Is this hard to do ?.... any guides or help you can offer would be greatly appreciated

Surveymonkey is one I've used in the past.
