Output a list of changes to image in photoshop

Hi,

Is there any way to output a simple list of what changes are made to an image in photoshop?
I'm considering entering the OcUK photo competition but would like to include a list of changes without having to enter them all by hand.
Any ideas?
The same qurestion goes for Gimp.
