They have given the power to fine people using cameras to councils, of course they are going to try it on and screw up. It's what they do.

More likely to be a faulty camera, rather than a conspiracy on the part of whoever operates the cameras?

Are you sure about this - I doubt councils have the ability to issue licence endorsements? Or are you talking about other things such as bus lanes?
