Passwords

For those who work within organisations, how do you have your passwords setup? For example, do you have all routers using one UN/PW, all servers using one UN/PW etc etc

i used to login as a domain admin -bad i know

each machine had seperate admin accounts though

all routers had the the same, to make it easier
