pc freeze with audio loop

evening guys,

i have found lots of similar issues on the net relating to the problem i am having, and it could point to any number of components.
it's happened twice atm over the last 3 days.

i have run prime95 for about 7-8 hours for the ram/cpu.
i didn't notice any issues.
prime95 doesn't automatically stop, am i correct, i would have to close it down myself?
what message should i be looking for that would indicate if the ram/cpu is failing using prime95?

i ran furmark in fullscreen 1080 / 2xmsaa for 30mins.
gpu temp: 83 average
gpu fans: 62% average

my psu reads 8.88v on the +12v, and it should be around the 11 or 12 mark shouldn't it?

my specs:

gtx 660
i3 cpu
evo labs 700w psu

i listed my specs at the end of my post, but i forgot to mention the mobo and ram, os!

4gb ram (well it's 32bit os, so around 3,25gb?)
 
any bsod? blue screen of death?

looked in event viewer to see what it crashed on?

and are you stock or overclocked? if overclocked how much cpu/vtt are you using

it would be worth testing with a spare psu if you have one or could borrow one
 
no bsod
didn't look in event viewer when both occasions it happened.
1st time:
netflix
2nd time:
youtube 720p60
so not exactly punishing stuff.

no overclocking
no spare psu and can't really borrow one.

any good deals with psu's atm?

someone advised me to get a new psu before i bought and installed my 660 2 months back.
didn't listen, as the psu appeared to be working just fine.
losing a third of its effectiveness from 12 to 8 doesn't look to good.

al i have from cracking the case and looking is this:

evo labs 750u switching power supply
intel 2.2 version p4
a sticker label reads 2009, so atleast six yrs old?

i used cpuid hwmonitor for the psu reading

edit:

gpu seems to be idling around 25 degrees

Netflix/youtube ect would push me toward memory or not enough vtt but its hard to say

with only 4gb id have thought you would have been fine but could try around 1.25v cpu/vtt and see if it still crashes
 
so set a custom test with prime95 tonight at 1.25v cpu/vtt?

how would i do that?
i just opened prime95, but couldn't see any way in which i could set it to test 1.25v cpu/vtt.

this is all first time stuff for me.
 
sorry I meant in the bios,look for cpu/vtt and manually set it to 1.25v save/exit

then use pc as normal or watch Netflix/youtube vids to see if it happens again,i doubt it would happen while stressing but could try a blend test
 
ok, i found a vtt profile setting in the bios, but it's greyed out.
i've looked around on the net with searches like "vtt settings greyed out in Gigabyte H61MA-D3V bios", and i cannot find out what it could be.

somebody did mention updating the bios to the latest version, as mine has not been updated since i bought it 2 years back.

i thought because it wasn't broken don't fix it.
