PC to TV

Hey, so I currently have a Sony KDL-40D3500 40" Bravia LCD TV in my room, and I'm wanting to connect my PC upto it. Just wondering what I need and where I would get it from.

I'm building a new rig at the minute, the card I'm looking at getting for it is the: Powercolor ATI Radeon HD 4870 1024MB GDDR5 if that helps anyway.

All I get from the brief on the TV is that you connect the PC with; PC Input (15pin D-Sub) + Audio In, so was wondering where I would get a cable like this from. Or does this not matter because of the card I have selected, does it have an HDMI port included in it? If so then could I just connect it to one of the HDMI ports on the TV, and run it through the HMDI channel instead of the PC channel like I do my PS3 that is currently using one of the ports.
 
Another question, the card I'm looking at, has two DVI ports if I'm not mistaken (Can't get a good shot of the rear of it), so I should be able to run a DVI-VGA adapter out of one DVI port for my monitor, and a DVI-HDMI cable to my TV. If I do this will both of them function at the same time, or will I have to switch from port to port somehow, I'm unfamilure with this sort of thing you see, lol.

Yes you can do what you said above, I do that as I have a High end 22" CRT and a 1080p TV.

No you will not need switch between the two, you can have both off/on or 1 off and other on all through the GPU Control Panel.
