Thats strange, it runs really smooth on my rig, which is similar to yours apart from gfx card. I didnt play with anything in the ini file, I found the H1Vltg3 Modified Settings worked fine. I think the first time I installed all the mods it didnt work so I went back and started from scratch, following the instructions exactly. Then it worked.
Also, it probably makes no difference but Im using the retail version, not steam.
I did however make adjustments in the ingame GTA4 graphics settings, I had to lower a couple things as the ENB pushes the fps down quite a bit. iirc it was the draw distance that robbed a lot of my fps by default.
Heres my ingame settings, not sure of the average fps, never ran fraps with it, but it runs smooth as silk.
