Persistent Chrome survey malware pop up

I've been having this survey pop up asking me to answer questions to win an iphone etc. for some time now and I have been unable to remove it. I have tried malwarebytes, hitman pro, various AV solutions, uninstalled and re-installed chrome but all to no avail.

Anyone could recommend something to remove this malware on chrome? It only affects chrome.

Check for any strange installed software. Probably a registry key some where in Windows if not.
