I use aero and it disables itself in games so it has no negative effect.
I use Media Centre all the time, I even have an MC remote.

You asked me to list "one thing" I use in Home Premium that's not in Basic, I listed four.

Search results in the Start Menu are not on Home Basic IIRC. I could easily be wrong.

me too, on my media centre pc
I have a HTPC downstairs. I use XP MCE + MC keyboard and remote on that.

My PC up here just happens to have the same functionality as my HTPC with the hardware to run games, so if I ever fancy watching a DVD or movie up here I just launch Vista Media Centre on my TV and either put the DVD in the PC or stream my movies from the HTPC downstairs. :)

Believe it or not some people do use their gaming machine for things other than games. Watching movies, listening to music, instant messaging, web browsing etc. are all pretty standard on any home machine these days and for the first two Media Centre just makes it a nicer experience. Especially if you have your TV connected to your PC (and there's no reason not to have).

And yeah, the basic Windows theme really would bug me that much. Even Luna looks nicer.

Your dealing in absolutes as if it will never be used for anything else as if it was to be installed like a kiosk.

And he said mainly for gaming & not absolutely as what would be the point in getting vista if all he is doing is gaming, is he using it for DX10.1 as all the other featured will not be used or needed

I assume you can connect to a workgroup with home premium?

I'm assuming he will use it for more than 'just' gaming, but the primary purpose will be gaming.

I have suggested he buy Home Premium 64 bit oem as 'ultimate' I really can't see being that more useful. I still don't understand the need for Microsoft to nerf certain versions though.

I use media centre on my machine because it meant I could pick up a cheap TV card and record from freeview. I then convert the files and watch them on my portable media player.

If the difference in price isn't great and this is something you might want to do it's pretty handy. Besides, your current gaming rig may become your HTPC next time it comes to upgrade.
