Power Supply RMA

Hi guys, my power supply has been causing issues recently such as my PC not starting and not being able to enter sleep mode, I've compared it with another functioning power supply and it does appear to be faulty so I'm sending it away to be repaired or replaced. I just wanted to know the procedure and what way I need to send it to OcUK so they know who it's from and so on. Thanks in advance!
 
Forgot to say do I need to send the manual and cable ties that came with it or just the modular power supply cables and the PSU itself? Should I just use a cardboard box or the packaging it came in? Sorry for all the questions first time going through the RMA process.
