Question regarding CVs

Hey, basically I need to do a CV (need a part time job) but I haven't done one in so long i've forgotten what to do.

Contact Details,
Information about myself,
Education,
Skills,
Personal Statement

Is there anything else that I've missed?
Also, should the CV be fancy or just basic and to the point?

What about format - should it be plain text or should I have colour etc in there?

I've put Curriculum Vitae at the top of the CV so far in a nice fancy font, I have a line running underneath it with my name above it, my contact details below it on the left and my address on the right.

The next bit should be a profile? A paragraph about me? - What should I include?
