Right, really peeved at this. Completed it twice now on realistic and still haven't been awarded the achievement for 'complete the story at the hardest setting' (or whatever it's called?)

It's an achievement glitch, it happened to a mate of mine, i also didn't get the Private First Class one.

It'll be sorted in the patch.

You should have done a google before going to the length of completing it again.

Have you checked that all the boxes are ticked to say that you have completed it in story mode?

Its a known glitch I think your screwed tbh. Forget about it.

It's a known glitch though can apparently be solved if you join the last mission as the second player in realistic, this should be patched soon (Whether it will or not is a bit =/)

 

[Scythe]

This is similar to the bug on Vegas 1, you chose Realistic but it left the settings at normal so you don't get the achievement.
