Random black screens can only get out by restarting.

I have an Infin8 Hunter custom build from OCUK.
My main display is a AW3423DW a second monitor which is an LG TV SSCR2 and a capture card that I stream to through a separate computer so I'm using 1 Display Port and 2x HDMI and completely randomly but becoming more frequent are black screens, the computer remains on and I can hear sounds still, only way to get it to come back is with a reset.

I've tried setting the Bios to default profile (removing all overclocks), fresh install of windows, safe mode DDU the graphics drivers and reinstall.
I did run a stress test to see if I could capture it happening and in that log file at point of failure the card just looks like it turns off, all voltages go to 0 and stays like that until I reset the machine.

Sometimes I do get an error in the event log but not always and the error is always like the 2 images below and always in that order, firstly the PCI bus failure and then the nvlddmkm failure.

I'm thinking this has got to be a GFX or power issue but before I can talk to OCUK on Monday does anyone have any ideas? I'm really looking for any reason to not send it back as I cannot lift this machine on my own.

I get the same, but I'll also get a green screen occasionally as well. I can't stay it happens too frequently (around 3-4 times in the last month), but it is a pain when it does. A couple of times, I get lucky and the driver will reset and the display will come back, but most of the time the screen will just stay black and will need to reboot to get the display back again.

I'll usually get the nvlddmkm error in Event Viewer :

"The description for Event ID 0 from source nvlddmkm cannot be found. Either the component that raises this event is not installed on your local computer or the installation is corrupted. You can install or repair the component on the local computer.

If the event originated on another computer, the display information had to be saved with the event.

The following information was included with the event:

\Device\Video8
Resetting TDR occurred on GPUID:100"

What GPU do you have? Like you, I've re-installed drivers using DDU, kept my GPU at stock to ensure it isn't an overclocked core or the memory that's causing the issue. I also have 2 monitors running as well. A regular 32" IPS panel and an LG C1 OLED.

At this point, I hope it's a driver issue that can be resolved through updates. From doing some digging around, people have said they are resolving the issue by going into the Nvidia Control panel and enabling "Maximum Performance" mode in the settings. It will draw a few more watts (I went from 38w to 58w), but they are saying that it resolved their problem and they no longer get black screens. It may have something to do with the way Nvidia are handling P-States with multi-monitor setups.

I get this any time I try to run a game about 5-15 minutes in.
I've tried back the driver to the latest signed driver from Windows and it's still the same.

I have a Asus TUF 4090 with an ek water block on it.

At this point the computer is pretty much unusable.

I thought this was interesting so I've been testing it this morning. I have had 0 crashes since only using a single monitor, unplugged the others entirely.
