Nope that's all you need, as all the other details they can track back through the network. It's nice and easy and assuming that you pass the data protection checks, theyll give you the 8 digit PUK within 2 mins or so

my girlfriend locked me out of mine :rolleyes:

phoned o2 and they had an automated out of hours system, might be worth checking if o2 do something similar
