Ripping a DVD as one file

Hi,
I'm trying to rip a DVD that is made of lots of chapters into one file to play on my xbox360 (the drive is dodgy as so only some discs play).
I've got K9 on the interwebs recommendation but this creates lots of chapters which I can't join up.
Anyone know how to copy as one file?
I did this on some DVDs before on my Vista unit before I switched.

DVD::Rip

Will rip and encode, should be in most repos.

Rip as an ISO ?
