RMA instructions

I asked OCuk to send me the RMA instructions to a different email but I don't think it worked. I have the RMA number so all I need is the address and like where to print RMA number etc. Can someone paste me this from a previous RMA email they've had? Thanks

the adress is the same as the invoice u got with the item and the code just goes on first line wit address directly below. also write it on both sides of package!
