Sending 360 back

My 2.5 year old 360 has finally died.

I have a shipping label from UPS but what box should I send it in?

Do you send them in their original box or find a different one?

Am I likely to get a HDMI system back?? Mines just component.

The screen started flickering on it just before it died so I guess it's the MOBO/GPU thats gone?

I didn't think MS bothered repairing your original console?

It has chopped a few discs up. I did the fault logging process online. How do I add the disc scratching fault??
