Sharepoint 2007

I know, I know, it's still 2007 and we're looking to upgrade to 2013, but in the mean time, I could do with some assistance around creating an interactive form on a SharePoint Webpart.

Basically, I'm looking to have a page showing a form which can be completed by the user and then email a specific email address with the push of a button.

I'm unsure on the webpart I would use for this and would welcome assistance.

Sounds like you just want to setup a basic list with the questions on it. Then adding a workflow to the list so when some submits the item it send a email with the added information to who you wants.

Another option would be to setup a infopath form but that would start to get more complex.
