You could turn the whole heatsink round, and then turn the fan round too.
I have done similar. Can you not also mount it 90 degrees out, your case has a top fan to remove the heat.
You can of course experiment! As long as the heatsink is attatched it should not overheat.
Use speedfan, try it with the rear fan + cpu fan. Then try it without the rear fan, then with rear but without cpu fan. You could even try adding a top fan into the mix and see what happens! I would add a top fan to the middle grill, but probably not the one at the back. I would be tempted to blank off the rear fan hole on the top to see if it increases airflow. (Try it with a book on top first, then use some coloured acrylic).
Remember to stress the CPU when checking temps. Idle means nothing in this game
