Splitting a 3.5mm combo jack?

Hello everyone, it is possible to split a 3.5mm combo jack?

I have a Plantronics headset with a normal 3.5mm combo jack which I want to connect a PC sound card with separate inputs for line out and mic in.

All the splitters I have seen work in the opposite way (separate plugs into a combo jack)
