SSD failing or other problem?

My gaming PC, (the ITX system in sig) has been having some BSD with the :( face the last couple days. Prior to the crash, the Teamgroup 480 SSD, keeps re-detecting and switching off the same way a USB device would. Clearly annoying when playing COD ,as it makes the New drive detected open up windows explorer. Its happened once when idlying but twice playing COD, COD isnt install on this drive either. I have checked the SATA and Power cables and all ok, although i will replace the SATA cable. Is this a sign of a failing SSD. motherboard problem? or something else
 
What error does the BSOD display? At the bottom it'll say a STOP code reason, usually that's a key indicator.

Often it can just be Windows/software/driver corruption, although it can also be hardware failure too... :(

I'd try reinstall latest chipset drivers, graphics drivers, check SSD firmware is up-to-date, move it to another SATA port (and new cable like you said if able), run virus/spyware/malware scans for any gremlins etc.
