SSD failure?

I've been having this issue off and on for a couple of months now and lately it seems to be increasing. I wonder if it's my SSD failing, but i'm not quite sure.

Computer will randomly freeze. Other times, it takes much longer than it should and used to coming out of sleep. I've also started getting BSODs. If I press reset, then the same thing happens. It takes a little bit longer than it should going past post and then it comes up with the error "Reboot and Select proper Boot device or Insert Boot Media in selected Boot device and press a key". Pressing a key results in the same message being repeated. If I keep pressing reset, then the same thing happens and it gets stuck at the same point. However, if I press the power button to turn it off and then immediately turn it on again, then it boots fine.

Anyone have any idea?

Spec is:

Samsung 250GB SSD 840 EVO SATA 6Gbs Basic - (MZ-7TE250BW)
Anidees AI-6BS Midi Tower Silenced Case - Black

One other thing which is HD related, not sure if that's the cause - I had a 1TB Samsung HDD in my old computer, which I transferred over to this one. Staright after transfer it kept coming up with error messages/HD failure, but worked fine for a year or two. Now it only randomly shows up in My Computer. Not really concerned with that HD, but mentioning only in case it helps diagnose this other error.
