Strange fan failure issue

Just had 3 fans fail on me at the same time, strange thing is one is a fairly new 140mm Scythe fan attached to a mobo header, the other 2 120mm Scythe fans are attached to case fan controller. Any ideas?

Tested the mobo headers and the fan controller connectors with other fans and they work fine. I'm unsure whether these Scythe fans have burnt out due to their power draw as I run them maxed out. Had a look around the net and seen a few other people say they've had slipstreams fail within months.
