regarding 6+2 pin power for gtx 480 graphics cards how many wires are supposed to be going into the connection, is it supposed to be 8 wires (one into each pin)?

Yes. It provides an extra 75w over a normal 6 way connector. So a 8 and a 6 pin provides 225w if power in addition to the 75w the pci-e slot provides.

Cable adapters that split a 6 way into a 8 pin connector are not really a good thing.
 
you'll have to forgive me as im quite rubbish at explaining things, but on the 8 pin to the 6+2 pin, on the 8 pin side which goes into the psu there is an empty space on the pin the empty space is across the same line where the yellow wires are goin in, and on the black wires there are 2 black wires goin into the one pin, if any of that makes sense

the psu is OCZ ModXStream Pro 700w Silent SLI Certified Modular Power Supply
 
Yes 2 neutrals direct from PSU. Quite how this is supposed to provide 75w extra is beyond me.

One of the wires could be snagged in the cable braid or something? Still is a bit sloppy by whoever made it.
 
This is nothing to worry about, on my corsair hx 850w, the blue 8 pin end that connects to the psu has a blank connector.

I can only assume that two wires emerge from one of the other seven connectors at the psu end. Ive checked all my pcie connectors and theyre the same.
