Stuttering of video/audio playback

I've got a problem it must be a rare issue because there is nothing in google about the problem I'm having.

Its a desktop PC the problem I'm having is some kind of timing issue with video and audio playback. For example if I play a DVD the audio will stutter and the picture isn't going at a steady rate, it don't matter if its a DVD or a video on the computer itself. It doesn't matter where the video is played from or the format, the same issue is there. If I play music the audio also stutters. I can not play most games because there is something wrong with the video speed rate and games either crash or do something funky.

I thought it was an issue with the video card at first but after changing the video card the problem is still there. I changed the RAM but it made no difference. The hard drive is in good working order and has been scanned. Power supply is good. The only thing that leaves is the processor but I doubt anything will change.

Has anybody encountered this problem?

Has anybody got any clue as to what this problem could be?
