Is it worth if and if so whats the best one to use?

Not worth it.

They do nothing you cannot do yourself, for free from within Windows directly or using free products with a bit of research on Google.

No real 'optimizers' as such. You could try Comodo System Cleaner or Ccleaner to rid your system of junk files [though personally I'd leave the registry alone], and defragment using Jkdefrag.

Also, use Winpatrol to reduce the amount of crud you have running at system startup. All these items are freeware, and frankly I doubt if there is any payware that could do any more for you.....

I normally just use Ccleaner and AusLogic disk defrag. normally gives me a bit of a boost.

You might want to give Ultimate Windows Tweaker (for Vista) a go to tweak it, rather than tune it...
