Looks like the plug connection wasn't great and it's arced inside the plug. No, I wouldn't use it.

Only had it just under three months, and the system wasn't exactly high performance (sister's internet PC).

Yup, RMA it as it's under warranty. There's no way that should have happened. Hopefully it hasn't damaged the motherboard socket.

Yeh return it, it might be a cheap ass psu, but that's dangerous. Could be an inherent design fault, it can happen, so the manufacturer and retailer need to know about it.
