Done. I think a 'Maybe' option would also have been appropriate for some of the questions rather than a fixed yes/no, but whatever.

Although as a bit of feedback, the questions/answers are very restricted and could be viewed as bias. Many of the answers would have a whole list of conditions for 'yes' or 'no' and few people would say either outright.

Secondly my course (and many others that I know of) don't require you to buy books (due to the content being spread over so many, the content not existing outside of academic papers or that many of the books are out of print etc). Also 90+% of my work is either typed or computational and all lecture notes are given in handout form, so my stationary expenditure is again minimal (I go to Rymans maybe twice a year at most). Not sure how you'd address this issue.
 
Done, although it took me quite a while due to the text colour - perhaps change it to something darker?
