Minerva is a really good single player mod.

There's also Black Mesa, which the original Half life game converted to the HL2 engine.
 
There were tons but by now most are dead in terms of multiplayer.

The hidden is amazing, the game is about a subject who escaped prison. He is invisible (like 90% invisibility, at cirtain light and angle you can sorta barely see the outline), he can jump very high stick to walls, even trow objects for physics kill. So like 8 solders with guns are deployed to kill him while the hidden must kill all of the soldiers. Tons of fun.

Battle grounds 2 was amazing too, similar to Medival Warfare but during the civil war in america, was tons of fun but now has become organized clans and line shooting (more historically accurate but boring in my opinion).

Zombie Panic - Another amazing multiplayer mod, starts off with 1 infected and however many survivors, the survivors must use planks to hammer in doors as well as furniture to block pathways or entrances, find various guns and survive the swarm of zombies, there are other levels where survivors must get to a certain point (from A to B) to be rescused, also tons of fun. This mod is actually stil fairly active if I am not mistaken.

As above poster has said, I can't imagine that many people play HL2 Mods anymore. But I used to play Insurgency which was a realistic modern military shooter set in the middle east. It was pretty good but I do not know if there is anybody playing it anymore.
 
Garry's Mod is a fantastic timesink. Used to spend hours upon hours in it both multiplayer and singleplayer, by far one of my favourite mods.
 
Research And Development was a fun mod for HL2.
Setting in the same universe but with no guns, just 100% puzzles.

Not sure it works with all the recent changes to the HL2 files though, its not been updated for a while.
 
Eternal Silence

It's a space combat game with infantry warfare mixed in. Each team has a number of battleships that you have to destroy. You have access to different spacecraft and there is different infantry classes with lots of different weapons to choose from


Synergy

Great mod I used to play, it's COOP where you do the HL2 missions, there are lots of custom maps too.

As already mentioned, The Hidden Source is fantastic. There are still a fair few playing, a few ocuk'ers played a couple of months back and there were still plently of full servers.

ZPS was also great fun from what I can remember, not sure how active that'll be now though
