Transfering domain names

Hi,
Is anyone here familiar with the transfer of domains?
We've tried to transfer a domain here, which we own, from one provider, to 123reg, but the process failed.
Do we need to do something at the 'from' provider?

And if so, what?
As their tech support isnt being all that helpful.
