ubuntu, icon theme messed up.

System -> Preferences -> Appearance,
Customize button,
Icons Tab,
Pick Human theme, or one you want,
Log out and back in.

If what you're not saying is you've screwed up your theme trying to mod it...
Open the Package Manager, Search for Human and Re-install Human theme and the icons.

Right click on it (Places) and select edit menus and click revert ?

I've never tried it though so will be interested to hear the result

no, i installed a new theme called meliae. but as above it shows the default gnome icons in the places menu, not anywhere else

The theme is not complete or not written correctly then.
If a theme is corrupt, badly coded, or does not assign all icons then the system uses gnome defaults.

If you've just applied it, try rebooting. If it still doesn't work then the theme file is not written correctly or not compatible with your current gnome build.
