Uneven floor under carpet at join

My house had an extension back in the 1980s. I purchased it 3yrs ago and I'm currently re doing the main bedroom.
The extension has a solid hardboard floor joining to floorboards in the original part of the room.

I noticed that the old carpet had a ridge in it, on lifting the carpet i can see why.
There is a slight lip in places where the hardboard meets the floorboards.
I've tried lifting the floorboards but they are well stuck down with lots of nails etc.

Is there anything I can use to get rid of this lip at the join? I was thinking of possibly using a piece of cardboard but any thoughts?
 
Get an orbital sander and smooth the transition or lift the ends of the floor boards to match packing underneath them.
 
Sander on the high point or Hardboard/thin ply on the low section. Both actions will depend on how big the area actually is, and how each part joins to the rest of the house!
