Hi Everyone 
I would appreciate your help with these two problems:
1
My father's PC has USB 1.1 on the motherboard so about 2 years ago we installed a USB 2.0 card. This card worked fine from the very start, however, now it has stopped functioning - the on-board USB 1.1 still seems to work correctly. The card's power supply appears to work as when a USB dongle is plugged in it lights up for a split second and then goes off again, also a USB "lava lamp" works. The card is not detected in the device manager and therefore I have tried uninstalling the card's drivers, then reinstalling them - no success. Also, after installing the drivers the PC blue screened and complained about an error with "ousbehci.sys". I have now removed the card from the system. The card does not appear to be damaged in any way.
Any ideas what could be causing this and how I could fix the problem?
Now for the other problem:

My father's PC will not reboot into Safe Mode. Originally it was thought that it was the USB Card which was stopping it but the problem still persists after the card was removed from the system. It is possible that the problem with safe mode has been there for a while as it has not been necessary to boot into it until now. When I try and boot into safe mode it will get as far as the login screen but before you have the chance to click a user name the PC reboots itself - no blue screen, it just goes straight back to the BIOS.
Any ideas on what could be causing this or how I could fix this?
The PC's specs are:
P4 1.7 GHz
512GB Ram - RDRAM (RAMBUS) NOT SDRAM
GeForce 3 64MB
Windows XP
The make of the USB 2.0 Card is Sitecom and the model number on the card is CN-014 VER:1.0
