VoIP Routers

My brain hurts, this is a very confusing topic, does anyone know how Voip routers work? I understand you can plug a standard phone into one and make calles over the Internet.

How does this work? With Skype you have an account and pay for calls that are made to a PSTN line (SkypeOut) but with a VoIP router there must be some sort of service provider I'm guessing. Anyone have a clue how to explain this because it seems a very confusing topic. I know I've heard SIP and FXS been mentioned and don't have an idea what thats about.

Can anyone explain how this works because I'm confused. This may not be availiable in the UK either, I just saw the reature on a router.
 
The Fritzbox is an excellent voip router with excellent firmware updates -Number one in Germany

My adsl router can also be used as a straight voip router and connected to a cable modem such as NTL

The router is the prefered method of voip usage as one does not need the puter to be on, also the router being first in the feeding line it can give priority to the band width needed by telephone calls being made at the same time as cruising the web or downloading files.
