How do I fix it? Specifically in Quake 4?

Without vsync i get horrible tearing...with it I get terrible mouse lag! I've tried forcing triple buffer...setting the prerender limit to 2....I don't know what else to do!

You'll have to put up with the tearing it looks like. Really, it's not that bad. Your 8800GTS should be running it with enough FPS with the V-Sync off to minimize tearing surely?.

Also it doesn't stop you killing, you can always see what you are shooting. I'd pick tearing over mouse lag any day. Then again I'm an on-line gamer mainly and V-sync = mouse lag and you need to be as sharp as you can be if you want to win or do well.

have you checked that your monitor is set to its maximum refresh rate, i was getting tearing in bioshock when extreme lit areas strobed and it was sorted with the refresh rate.
might be worth a look

My monitors max refresh rate is 60hz...and what I don't understand is that with my 7900GT I had vsync on and there was no mouse lag.

im not really sure...its been a long time since i played an opengl game online.
i played rtcw for some years and that was based on the quake engine...ive had mouse lag but cant remember what the cause was lol.

but some things to try

make sure mouse acceleration is disabled in mouse software...as that can cause a lagging effect.

disable mouse acceleration in game which also can cause mouse lag...cl_mouseaccel 0
i think thats the command...should be same for quake and rtcw

if that doesnt work then try disabling mouse smoothing...m_filter 0

hope it helps

It's just the nature of vsync.

Your best bet is to try and eliminte the tearing but experimenting with different framerate caps. For example 60fps @ 60hz is likely to cause tearing. Try capping it to a higher value if that's possible in Q4.
