Tried to narrow this down, I only have the one monitor to test, but same happens with two separate displayport cables. Monitor is Dell S2716DG, a few years old at this point now.
To fix it I simply have to unplug then reconnect the displayport cable. Additionally, waking from sleep can sometimes cause the image on the display to become slightly shifted, almost a few pixels with to the right, same fix as above.
I'm thinking monitor issue, the GPU is fine in everything else, but just wanted to see what others thought?
