What do you use for your document management?

Currently considering alternatives to our vendor specific practice management software since they are EOL their software in a year, current option is to move to their new product, get a developer in or look into something similar. I am really struggling to find something to replace what we currently use.

We are a practice,
  1. the PA's need a method to create documents (using a template that pulls correspondence details from a DB) and lookup client details / reference numbers
  2. the users need to be able to file their emails into respective client folders
  3. there needs to be a way to see which documents were created recently for a respective client.
  4. the ability to scann correspondence / documents into client folders

Would be interesting to see what you guys use.
 
Documentum & Captiva (Captive for number 4 in your requirements).

Enterprise software, so might not be appropraite for you - no idea of your size.
 
Alfresco. Agile and free (if you can spare the development time!) - else their Enterprise offering is brilliant, apparently and plenty of consultants working with it - exponential growth!
