Sorry for being a complete idiot, but I have no idea what this is called, and need to buy a replacement.

Not knowing what it is called is proving to be the stumbling block to getting another one.

Is there a similar device at the other end of the cable coming out of the top? It looks like a splitter for running 2 separate ethernet runs down a single Cat5e cable - I forget what the proper name is though...
 
CAT5 Splitter mate, though if you buy a replacement you'd be best off buying them in pairs as different ones aren't always wired the same.
