What Software Is Used to Make Surveys?

I need to make a few surveys for university, when I make them in word they just look, well appalling. Is there any half decent software that is for making surveys, if yes what are they? Google just beings up websites like survey monkey

Electronic surveys or surveys to be printed out?

MS excell and Word will do the trick in either situation

Surveys to print out, word doesnt seem to do scales very well for example Likert Scales
