This sounds exactly like what happens if you try too hard and insert a molex comnnector the wrong way round. I managed it once reaching under a dark desk to connect a HDD.

That PSU appears to use molex connectors at the PSU end of the modular cables so it could have got swapped.

that would make sense but no cigar as the shape doesn't allow you to swap them around the wrong way no matter how hard you try.

It depends how soft the plastic used to make the plug/socket are. I’ve also seen some variation in the actual plugs; some combinations can be difficult to connect and disconnect, and others almost fall out on their own.

I've managed to reverse connect a molex once myself (in the 20 years I've worked with PCs), and I've seen the results after other people have done it several times.

If your sure nothing was reversed then you had a very strange problem and the bin is probably the best place for it.
