What to do with this

At the moment i have this form in the making

I know it has a lot of boxes that need filling in.
In the past i have used forms to email people, but this looks like it will send a complicated e mail if i choose to go that route,
If you was to do this how would you go about it, and getting the information?
also can i use Style sheets to make it nice?

i dont really understand what you're saynig, this is a form.

yeah in the past i have used forms to send there data as a email, so it would say eg textname = matthew
What im saying is would i be better off using php or something to get the data from the form and display it in another way?

yes, if you what you want from the form when filled out is for it to be sent you you then php is what you're after to do this.
