What exactly do you mean by a "custom" email address?
If you want one on a personal domain name you'll have to purchase the domain name yourself, and some web hosting/email package. If you just want an email address on a domain which is slightly irregular, there are hundreds of places out there where you can get such email addresses. What's wrong with hotmail, gmail etc. anyway?
I wouldn't really call those custom e-mail addresses, surely? Custum to me = not like anyone else. If that is what you mean then I can tell you how I got mine setup.
Bought domain from www.ukreg.com. Use www.zoneedit.com for my DNS servers, though you can use the ukreg services the control panel is not so simple. Then for hosting - I have a static IP (well, several actually but you only need one) - which the MX records point to. At my end of it all, I run a Windows XP box with the free version of Mail enable from www.mailenable.com. That's one way of doing it anyway - I hope in the near future to move over to a Linux setup, so that I can a) change to an IMAP inbox (POP3 atm) b) Get Squirellmail setup locally, since my school seem to find it funny blocking www.mail2web.com
Be careful of companies offering free domains. There is usually a catch, I came across one that charged £100 if you wanted to transfer the tag to another host once you outgrew/got bored with with their crappy service.
