Why do some photos get mixed up?

I can see the photo fine as thumbnail in Windows however when its opened by Picasa/IE/Office Picture Viewer, its all mixed up.

This doesn't seem to be specific to a particular camera I own as its happened a few times to all of them. I'm starting to think its the XD and SD cards I'm using. But then why does the thumbnail appear fine? I've tried opening the pics on XP and 7.

Weird. Almost looks like 80's photos stacked on top of one another dont it?

Yeah does look a bit 80s, could be a cool effect

It looks like file corruption in the JPEG but normally that will have a more obvious effect on the bottom half of the image. Probably worth trying a different card.

Thumbnails are fine because its a separate file of the image so that could be saved fine but the actually large image is saved on probably a bad part of the card
