Why do systems 'go down' or are very slow?

I work at a council and we use Windows 8.
A week ago a Windows security update made all but our C drives disappear meaning we couldn't do any work for the rest of the day. No access to emails or the databases we use.

This was an issue with SMB shares.

Microsoft really need to sort their act out when it comes to testing.
 
Lack of investment in the IT infrastructure and poor maintenance of the systems - this can apply to any sized business (particularly the lack of investment)
Daft users running poorly optimized queries on the DB is a good one
Month-end or other periodic jobs running

Poorly architected systems and any combination of the above - one company I worked at we had to disable access to the B2B Customer Portal whilst we executed the bi-monthly Bill runs - wouldn't have been a massive issue if the Bill run didn't take 3 days to complete :rolleyes:
 
In my experience with systems and places i have worked a lot of the trouble is the IT staff simply either being to few, not skilled or experienced enough or simply not giving a toss.
Add in a healthy does of stupid management everywhere and the inevitable lack of funding for IT services as its seen as a cost center and thus never gets enough money and you have a perfect storm of **** ups all the time.
