Windows Phone 8.1 account issues

We have about 40 Windows Phones at work.
Each one has an Outlook.com account set up for it so the user can download apps.
The account is not used for any email.

We keep getting issues where the account requires verification, user agreements accepting or passwords changing.
You log in to the account on Outlook.com and it wants to send a PIN to the handset.
Sometimes it then requests the password reset.

It's getting to be a major pain to support. Any ideas WHY it does this?
One of the biggest issues it it also causes one of the installed apps to crash until the account is 'unlocked'.

Part of it seems to be related to how often the phone 'connects' to the internet. Some are used in very rural areas and have no consistent data signal.

Before anyone asks we don't give the users the Outlook.com account details. Trust me it would cause more problems than it solves!
