So when you've finished the game and can replay any chapter, does choosing the Uber difficulty for each chapter constitute finishing the game on Uber and therefore earning the trophy?

Or do you have to start from scratch which means all your perks are reset.

Interesting. There seems to be more to The New Order than meets the eye. If you play one timeline, it seems you can lockpick doors but if you play the other one, you can't lockpick but you can hotwire things.
