XBMC run at startup and menu setup?

Can someone explain how to get a Windows XBMC install to boot at startup?

Also, what's the best skin around and how does one setup menus such as:

Movies
Tv Series
Music
Pictures

etc...

Anyone know how I can boot xbmc live on a Macbook with rEFIt (dual booting OS X and Windows 7)?

You can't set boot priority on a MacBook EFI bios can you?
